Job Summary
Occupational Therapist Home Health (OT HH) position is available in a healthcare provider's home health setting. The role involves providing therapy services to geriatric patients, with a focus on activities of daily living, biofeedback, cardiac rehabilitation, and various orthopedic and soft tissue injuries. The position offers a flexible schedule Monday through Friday with potential weekend work, serving a territory within a 60-mile radius from the hub.

Job Details

• Required Qualifications: Valid license in hand; at least 1 year of recent relevant experience; proficiency in Homecare Homebase (HCHB) Electronic Medical Record system required; minimum of 1 year of relevant experience; OASIS experience required for staff completing Start of Care; timely communication and documentation required; active driver's license and personal transportation required; certification in Basic Life Support (BLS).
• Schedule: Dayshift, 8 hours per day, minimum of 40 hours per week.
• Additional Job Details: Home health occupational therapy focusing on geriatric outpatient care; the position includes responsibilities such as charting, patient assessments, and therapy planning; orientation required on Mondays; on-call rotation may be required; effective communication and documentation are mandatory; candidates must have an active drivers license and own transportation; travel Pairs accepted but each must travel within own territory with own transportation; modules are billable and completed during orientation.
