Job Summary

Physical Therapist, Outpatient Crestview, Florida. The Physical Therapist evaluates assigned patients and develops, implements, and modifies individualized treatment plans in accordance with patient needs, goals, and physician direction. The role provides comprehensive outpatient therapy services, patient and family education, clinical documentation, and support for safe, effective return to community activities.

Job Details

• Required Qualifications: Degree in Physical Therapy from an accredited school; at least 1 year of outpatient clinical experience; Cardiopulmonary Resuscitation (CPR) certification; active and applicable license for the state where services are performed.
• Schedule: Day shift, 8 hours per shift, 40 hours per week.
• Responsibilities: Complete comprehensive patient evaluations; use clinical reasoning to establish treatment plans and goals; provide discipline-specific therapy services; document care accurately and timely; ensure treatment is provided under valid physician orders; participate in student education and supervision as assigned; maintain professional licensure and required continuing education; follow applicable state practice acts, regulatory requirements, safety procedures, and professional standards; contribute to quality and performance improvement initiatives.
