Job Summary

Physical Therapist, Outpatient Queen Creek. The Physical Therapist evaluates assigned patients and develops, implements, and modifies individualized treatment plans based on patient needs, goals, and physician direction. The role provides outpatient therapy services, supports functional recovery, educates patients and families, and promotes safe, high-quality patient care.

Job Details

• Required Qualifications: Degree in Physical Therapy from an accredited school; 1+ year of outpatient clinical experience; Cardiopulmonary Resuscitation (CPR) certification.
• Schedule: Day shift, 8 hours per shift, with a minimum of 40 hours per week.
• Additional Information: Responsibilities include completing comprehensive patient evaluations; developing treatment plans and functional goals; documenting care accurately and promptly; providing services under valid physician orders; educating patients, families, students, and colleagues; participating in student development when assigned; maintaining professional licensure and continuing education; adhering to applicable state practice requirements and professional standards; and contributing to quality improvement initiatives.
