Job Summary

Physical Therapist, Physical Therapist Outpatient Peoria. The outpatient physical therapist evaluates patients and develops, implements, and modifies individualized treatment plans based on patient needs, goals, and physician direction. The role provides orthopedic-focused care, supports functional recovery, and delivers education to patients, families, students, and coworkers.

Job Details

• Required Qualifications: Degree in Physical Therapy from an accredited school; at least 1 year of outpatient clinical experience; current Cardiopulmonary Resuscitation (CPR) certification; active and applicable professional licensure in the state where services will be performed.
• Schedule: Day shift, 8 hours per shift, with a minimum of 40 hours per week.
• Additional Information: Responsibilities include completing comprehensive patient evaluations; establishing treatment plans and functional goals; providing discipline-specific therapy services; documenting evaluations, treatment plans, progress, medical necessity, and charges accurately and timely; ensuring care is provided under valid physician orders; participating in student education when assigned; pursuing continuing professional development; adhering to applicable state practice acts and regulatory requirements; and supporting patient and employee safety practices.
