Description

Job Duties:
Assess patients to determine mental, physical, and spiritual needs.
Assesses, plans, implements and evaluates an individual plan of care in collaboration with the multidisciplinary health care team. Required:
Current Illinois Registered Nurse license
Current CPR/BLS Card
ACLS
