Description

Job Description: Assist the Director of Social Services in developing, implementing and periodically updating the written procedures for the Discharge Planning Program. Assist the Director of Social Services in planning the Nursing Services portion of the resident's discharge plan. Participate in audits of clerical records as designated by Nursing Administration. Required Certifications: BLS Required
