Description

Shift: Days (0800-1630), no weekends

Work Experience:

  • Assess Patient Needs
    Evaluate patients’ emotional, social, financial, and environmental needs to develop appropriate care and discharge plans.

  • Provide Counseling & Support
    Offer emotional support and short-term counseling to patients and families coping with illness, hospitalization, disability, or end-of-life issues.

  • Coordinate Care & Community Resources
    Connect patients with community services such as home health care, rehabilitation, financial assistance, housing, and support groups to ensure continuity of care.

  • Advocate & Document Services

License:
Oregon LCSW (Licensed Clinical Social Worker)    Certifications:
BLS/CPR Certification.
