Description

Shift: Monday-Friday, 7:30am-4pm, no weekends, holiday rotation

Work Experience:

Evaluate and treat patients in the acute care and nursing home settings. Also screen patients and attend meetings regarding patient progress.
Documentation of evaluation, progress notes, daily notes, insurance, and discharge notes in patient's medical records.
Collaborate with team members to ensure effective carryover of services for patient care.
Outpatient evaluation and treatments.
1+ years of related work experience required

Education:
Master of Occupational Therapy required License:
National Board Certification in Occupational Therapy and Wisconsin occupational therapy license Certifications:
BLS/CPR Certification.
