Description

Shift:
Day 4x10-Hour (07:30 - 17:30) Work Experience:

Conduct client assessments
Evaluate clients’ mental health, social, and environmental needs to determine appropriate care and support plans.
Develop and implement treatment plans
Create individualized care plans and provide counseling or therapy (individual or group) based on client needs.
Provide case management & support
Help clients access community resources, coordinate services, and guide them toward achieving personal and social goals.
Collaborate and maintain documentation

License:
MSW LMSW Certifications:
BLS/CPR Certification.
