Description

Shift:
Day 5x8-Hour (08:00 - 16:30) Work Experience:

Assess patients’ social, emotional, and environmental needs and develop individualized care or support plans.
Provide counseling, crisis intervention, and emotional support to individuals, families, and groups facing medical, behavioral, or social challenges.
Coordinate community resources, referrals, discharge planning, and support services to ensure continuity of care.
Maintain accurate case documentation, advocate for clients’ rights, and collaborate with healthcare providers, agencies, and multidisciplinary teams

License:
LCSW Certifications:
BLS/CPR Certification.
