Description

Shift:
9:00 AM-5:00 PM Work Experience:

Conduct psychosocial assessments for obstetric, gynecologic, postpartum, and high-risk pregnancy patients to identify emotional, social, financial, and safety needs.
Provide crisis intervention, supportive counseling, and referrals for concerns such as perinatal mood disorders, domestic violence, pregnancy loss, substance use, adoption, and child protective services when appropriate.
Coordinate discharge planning by arranging home health services, community resources, maternal and infant support programs, behavioral health services, and follow-up care for mothers and newborns.
Collaborate with obstetricians, nurses, neonatologists, case managers, and the interdisciplinary care team while maintaining accurate documentation and ensuring compliance with hospital policies and regulatory standards

License:
Valid Licensed Master of Social Work (LMSW) license issued by NYSED Certifications:
BLS/CPR Certification.
