Job Title: PT (Physical Therapist)
City/State: Dickinson, North Dakota
Shift: 8H Days M-F, 8a-5p - no weekends
Contract Length: 91 weeks

Job Description:
Seeking a Physical Therapist for a home health department. This role involves managing patient care plans, performing start of care and recertifications, and conducting regular patient visits. Candidates must have a current driver's license and auto insurance, and must follow a radius rule of 50 miles for employment eligibility.
Responsibilities:
- Perform start of care and recertification visits.
- Conduct 5-6 daily visits without additional travel or admission assignments.
- Provide safety evaluations or teaching visits in hospice situations if needed.
- Work collaboratively with support from the Clinical Coordinator and Director.
Requirements:
- Current BLS certification and ND Physical Therapy License.
- Preferred experience in Home Health and OASIS training.
- Experience managing a workload of 6.5 points per day.
- Black, navy blue, or grey scrubs are required.
