Job Description:

Responsible for screening, evaluation, treatment, and discharge planning for patients in inpatient, Long Term Care, Alzheimer's, Home Health, and outpatient settings. Collaborates with interdisciplinary treatment team.

Responsibilities:

• Manage patient caseloads in all clinical settings from screenings and evaluations through discharges.
• Compliant with State and Federal Regulations.
• Active participant in Interdisciplinary Team approach to patient care.
• Provide necessary documentation for billing purposes.
• Communicate effectively with physicians and rehabilitation team regarding treatment plans and patient responses.
• Counsel patients and families according to the degree of disability or impairment.
• Complete all required documentation thoroughly and in a timely manner.
• Participate in educational programs and in-service meetings.
• Maintain established policies and procedures, quality assurance, safety, environmental, and infection control standards.
• Attend meetings and in-service sessions as directed.
