Job Description:

Responsible for screening, evaluation, treatment, and discharge planning for patients in inpatient, Long Term Care, Alzheimer’s, Home Health, and outpatient settings. Collaborates with interdisciplinary treatment team.

Responsibilities:

• Manage patient caseloads in all clinical settings from screenings and evaluations through discharges.
• Compliant with State and Federal Regulations.
• Active participant in Interdisciplinary Team approach to patient care.
• Provide necessary billing documentation for billing purposes.
• Communicate effectively with physician, other members of rehabilitation team and others as indicated regarding treatment plan and patient response to plan.
• Counsel patient and family according to degree of disability or impairment.
• Complete all required documentation thoroughly and in a timely manner.
• Participate in educational programs and in-service meetings.
• Maintain established policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards.
• Attend meetings and in-service sessions, as directed.
• Require regular attendance and punctuality.
• Perform other duties as assigned.
