Job Title: PT (Physical Therapist), Rehab, PT

Shift: 8H Days M-F, 8a-5p + rotating weekends

Job Description:

This role involves providing comprehensive physical therapy services to patients within a home health setting, ensuring optimal rehabilitation for conditions such as CHF, Ortho, and Pneumonia.

Responsibilities:

Conduct "start of care", recertifications, and daily visits averaging 6.5 per day.
Cover home health areas and handle a range of common diagnoses.
Interact with the clinical manager or educator for support in emergencies or during regular hours.

Requirements:

BLS Certification
Preferably experienced therapists, not necessarily new grads
