Job Description:

Provides professional nursing care to patients across the age spectrum within an assigned unit. Supervises and coordinates care planning with other disciplines. This position is represented by RCHEA.

Responsibilities:

• Assesses patient needs, problems and concerns.
• Performs data collection in an on-going systematic manner, focusing on physiological, psychosocial, cultural, spiritual, and cognitive status, including nutrition, pain, patient/family education, family involvement, and patient advocacy.
• Evaluates effectiveness and outcomes of care given, and adjusts plan of care process and provision as necessary with consultation as needed.
• Demonstrates succinct, legibly written documentation skills on all medical records and forms.
• Navigates electronic medical record system to document completely and precisely.
• Participates in nursing research activities as requested.
• Formulates a goal-directed plan of care that is prioritized and based on patient diagnosis and anticipated patient outcomes.
• Implements care in a thorough, safe, skillful, consistent and continuous manner.
• Delegates appropriately and coordinates duties of healthcare team members.
• Ensures compliance of team with applicable laws, rules, and regulations.
• Communicates effectively with patients, families, and members of the healthcare team while upholding a positive professional manner and service philosophy.
