Job Description:

The physical therapist (PT) delivers quality therapy services to patients by performing evaluations and developing individual treatment plans and goals to maximize functional outcomes. The PT is responsible for maintaining standards of professional and ethical practice following the guidelines of the State of Texas Board of Examiners for Physical Therapy. The PT actively participates in discharge planning, implements the treatment program, and reassesses treatment results. Services are delivered within the framework of an interdisciplinary team approach, including significant others, and the patient. These services are delivered in compliance with CHI St. Joseph Health's mission, ethical standards, and policies and procedures.

Responsibilities:

• Perform patient evaluations and develop individual treatment plans and goals.
• Actively participate in discharge planning and implement treatment programs.
• Deliver therapy services in an interdisciplinary team setting.
• Float between inpatient and outpatient care.

Requirements:

• BLS Certification
• Texas and national PT license
• Wound care experience preferred, but not required
• IP/OP experience
