Job Title: YOSVH Rehab PT – LTC
City/State: Hilo, Hawaii
Shift: Variable - 7 days per week departmental coverage - 5 day work week. Day shift, 4x10 Flex, 07:00:00-17:00:00
Contract Length: 13 weeks
Start Date: 09/14/2026
Job Description:
Provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF) as a Physical Therapist. Responsibilities include performing evaluations, reassessments, and treatments consistent with licensure, discipline, and scope of practice.
Responsibilities:
- Perform evaluations, reassessments, treatments, and other clinical services.
- Develop and implement individualized treatment plans in collaboration with supervising therapist, physician, and team.
- Implement evidence-based therapeutic interventions.
- Educate residents, families, and caregivers on treatment plans and progress.
- Complete accurate documentation according to facility, state, federal, and payer requirements.
- Supervise therapy assistants when applicable.
Requirements:
- Minimum two (2) years of experience in the SNF/ICF setting preferred.
- Current Basic Life Support (BLS) certification.
- Current State of Hawaii license in good standing appropriate to the discipline.
