Physical Therapist (PT)

Job Description

We are seeking a dedicated Physical Therapist (PT) to join our home health team. The ideal candidate will have experience providing care in diverse settings and will demonstrate excellent communication and organizational skills.

Responsibilities

• Provide physical therapy treatments in patients' homes, assisted livings, senior apartments, and board and care homes.
• Manage a caseload of 3 to 5 patients per day, including 2 Start of Care visits and multiple evaluations.
• Complete comprehensive documentation and case management responsibilities.
• Supervise up to 2 Physical Therapist Assistants (PTAs) or Certified Occupational Therapy Assistants (COTAs).
• Collaborate with the RN Manager and Regional Rehab Leader for therapy-related inquiries and support.
• Adapt to varied home environments, providing care in potentially challenging conditions.

Qualifications

• Current Physical Therapist license in California.
• 2 years of experience in a healthcare setting, with a preference for community or home health experience.
• Experience with EMR systems, specifically HomeCare Home Base (HCHB), is preferred.
• Knowledge of home health regulations and documentation requirements, including OASIS.
• Current Basic Life Support (BLS) certification.
• Valid driver's license and proof of automobile insurance.

Work Environment

The position involves working primarily in patients' homes, covering various territories based on patient needs. The schedule is Monday through Friday, with occasional weekend and holiday shifts. A three-week orientation process will be provided, including online training and precepting in the field.

Dress Code

Business professional attire is required, including closed-toe shoes. Scrubs are recommended but not provided. Candidates should avoid jeans, tank tops, or shirts with logos.

Additional Information

This position requires flexibility in covering patients across different teams and areas as needed. Mileage reimbursement is available with prior approval, ensuring an efficient and organized approach to patient care.
