Job Description

We are seeking a dedicated Physical Therapist (PT) to join our team primarily working in a skilled nursing facility, with opportunities to float to an inpatient acute care location within the same premises. Our environment focuses on delivering quality care to a diverse patient population, including geriatric patients with chronic conditions and those recovering from surgical procedures.

Key Responsibilities

• Provide physical therapy services to patients, primarily in a skilled nursing setting.
• Evaluate patient progress and modify treatment plans as necessary.
• Work closely with an interdisciplinary team to ensure comprehensive patient care.
• Perform patient transfers and assist with mobility as required.
• Educate patients and their families on treatment plans and rehabilitation strategies.

Qualifications

• Valid California Physical Therapist license.
• Current BLS certification (AHA).
• Minimum 6 months of experience in either acute care or skilled nursing, with a preference for skilled nursing experience.
• Strong communication skills and ability to work independently.

Patient Demographics

Our patient population includes individuals recovering from various medical conditions, including:

• Total joint replacements (TKA, THA)
• Stroke (CVA)
• Deconditioning and pneumonia
• Balance difficulties associated with general medical conditions
• Chronic obstructive pulmonary disease (COPD)

Work Environment

This position will primarily involve weekday hours with a requirement for two weekend shifts per month. The facility operates with a patient-centric focus, offering a supportive atmosphere for both staff and patients.

Charting System

Documentation will be conducted using the Cerner charting system.

Cancellation Policy

The facility reserves the right to cancel shifts as needed, up to four shifts for assignments longer than 13 weeks, and three shifts for assignments shorter than 13 weeks.
