Job Overview

We are seeking a dedicated and experienced Physical Therapist to join our team in a Skilled Nursing Facility setting. This position offers the opportunity to work exclusively on Saturdays, with a focus on providing high-quality care to residents in a supportive environment.

Key Responsibilities

• Conduct comprehensive evaluations and assessments of patients' physical abilities and needs.
• Develop and implement individualized treatment plans to improve patients’ mobility and quality of life.
• Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes.
• Monitor and document patients’ progress, modifying treatment plans as necessary.
• Educate patients and their families on rehabilitation techniques and home exercise programs.

Qualifications

• New York State Physical Therapy License is required.
• Current Basic Life Support (BLS) certification is required.
• Prior experience working in a Skilled Nursing Facility is required.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.

Work Schedule

This position requires availability on Saturdays from 8:00 AM to 4:30 PM.
