Job Summary

We are seeking a dedicated and skilled Physical Therapist to join our team. This position offers a weekday schedule with no weekend shifts and requires the ability to work during regular business hours. The Physical Therapist will provide patient care in a home health environment, utilizing specialized knowledge and training in physical therapy practices.

Key Responsibilities

• Conduct assessments and develop individualized treatment plans for home health patients.
• Utilize HCHB and OASIS documentation systems effectively.
• Ensure compliance with all relevant regulations and standards of practice.
• Monitor patient progress and adjust treatment as necessary.
• Collaborate with other healthcare professionals to ensure comprehensive care.
• Maintain accurate and timely documentation of patient interactions.

Qualifications

• Doctor of Physical Therapy (DPT) degree, required if licensed on or after January 2016, or foreign equivalent.
• Minimum of 1 year of home health experience.
• Physical Therapy licensure in all applicable states of practice, required.
• Current CPR or BLS certification from an approved vendor, required.
• Valid Driver’s License and must be at least 21 years of age or older, required.
• Experience with EPIC electronic health records is preferred.
