Description
Job Summary
We are seeking a dedicated Physical Therapist with experience in home care and home health to provide quality patient care. The role involves a variety of responsibilities including evaluations, start of care (SOC), recertifications (ROC), and daily visits to patients in their homes.
Key Responsibilities
- Conduct patient evaluations and assessments.
- Provide start of care and recertification visits.
- Deliver therapy services to patients with diagnoses such as Congestive Heart Failure (CHF), orthopedic issues, and pneumonia.
- Maintain accurate and timely documentation of patient care.
- Collaborate with the clinical manager or educator for support while in the field.
Required Qualifications
- Current Physical Therapist license
- Basic Life Support (BLS) certification
- Experience in home health and home care settings
Work Schedule
The position involves working five 8-hour shifts, with expectations of weekend rotations and participation in holiday rotations.
Orientation and Training
New hires will receive two weeks of orientation, which will include shadowing and visits with a work coach.
Additional Information
- Scrubs or business casual attire is acceptable.
- Company-provided phone and tablet for work purposes.
Location
This position covers home health services in the community.
