Position Overview

The Physical Therapist in Home Health is responsible for delivering high-quality physical therapy services to patients in their homes. This role involves evaluating patient needs, developing individualized treatment plans, and implementing therapeutic interventions to improve the patient's physical function, mobility, and overall quality of life.

Key Responsibilities

• Conduct patient assessments and develop comprehensive treatment plans.
• Perform start of care, recertifications, and resumption of care visits.
• Maintain a caseload of approximately 5-6 patients per day, adjusting for admissions as necessary.
• Collaborate with clinical coordinators and other healthcare professionals to ensure optimal patient care.
• Provide education and support to patients and their families regarding home exercise programs and physical therapy interventions.

Qualifications

• Current licensure as a Physical Therapist in North Dakota.
• Basic Life Support (BLS) certification.
• Prior home health experience and OASIS training are preferred.
• Strong communication skills and ability to work independently in a home care setting.
• Must have a valid Driver's License and Auto Insurance.

Work Environment

This position operates primarily Monday through Friday from 8 AM to 5 PM, with no weekend or holiday requirements. The therapist will be expected to work closely with other healthcare professionals and have access to clinical support as needed.

Orientation and Support

New hires will receive 1-2 weeks of orientation under the supervision of an experienced physical therapist to ensure a smooth transition into the role.

Attire

Professional attire includes black, navy blue, or grey scrubs.
