Job Description

We are seeking a dedicated Physical Therapist (PT) to provide high-quality physical therapy services to patients in the Yolo County area. This position involves evaluating and developing customized treatment plans based on physician orders and patient needs.

Unit Information

• Unit Type: Home Health PT
• Shift Length: 8 hours
• Guaranteed Hours: 40 hours per week
• Typical Shift: 8:30 AM - 5:00 PM, with one Saturday per month

Responsibilities

• Provide physical therapy services to patients according to a physician's plan of care.
• Assess, evaluate, and update therapeutic status; participate in developing total care plans.
• Evaluate home environments and assess patient mobility and functional ability.
• Instruct patients and families in the use and care of therapeutic appliances.
• Monitor and report on patient reactions to treatment and changes in condition.
• Initiate therapy programs and instruct caregivers on ongoing patient care interventions.
• Train patients in the use of prosthetic devices when necessary.
• Supervise Home Health Aides participating in the rehabilitation process.
• Prepare clinical and progress summaries and maintain accurate patient records.
• Provide consultations to families and caregivers as needed.
• Participate in peer consultations and ongoing staff competency assessments.
• Comply with healthcare regulations and licensure requirements relevant to the position.
• Perform other duties as assigned or requested.

Qualifications

• Minimum of 2 years of experience as a Physical Therapist in acute, home care, or rehabilitation settings.
• Required Certification: BLS Certification.
• Strong oral and written communication skills.
• Good organizational skills and the ability to work effectively in a team.
• Scrubs are not required; appropriate professional attire includes slacks and a polo or button-up shirt.

Service Area

The primary service area for this position includes: Woodland, Davis, Yolo, Dunnigan, Arbuckle, West Sacramento, Natomas, Knights Landing, Capay, Vacaville, Winters, and Esparto.

Compensation

Mileage reimbursement is provided at the GSA rate with prior approval.
