Job Description

We are seeking a dedicated and experienced Physical Therapist to join our team in a Skilled Nursing Facility. This position is essential for providing quality care and rehabilitation to our patients.

Responsibilities

• Conduct comprehensive assessments of patients' physical therapy needs.
• Create personalized treatment plans based on assessment results.
• Implement physical therapy interventions and monitor patient progress.
• Collaborate with a multidisciplinary team to optimize patient outcomes.
• Educate patients and their families on rehabilitation strategies and exercises.
• Maintain accurate documentation of patient care and treatment plans.

Qualifications

• Current New York Physical Therapy license is required.
• Basic Life Support (BLS) certification is required.
• Prior experience working in a Skilled Nursing Facility is mandatory.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively within a team environment.

Schedule

This position is full-time, Monday through Friday, with a shift from 8:00 AM to 4:30 PM.
