Patient Types/Common Diagnoses Seen

Physical therapists will work with a diverse range of patients, including:

• Medically compromised patients: Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), Diabetes Mellitus (DM), Cardiac conditions
• Orthopedic and neurological patients, dementia, and general weakness
• Patient ages ranging from early adulthood to geriatrics

Required Certifications

• South Dakota licensure
• Basic Life Support (BLS) certification

Required Education

Bachelor's degree in Physical Therapy.

Skills Required

Candidates should possess strong general physical therapy skills, including:

• Patient evaluation and assessment
• Development of effective intervention strategies
• Medical assessment skills and care plan development

Departmental Support

The Home Health Department team includes:

• Registered Nurses (RN)
• Licensed Practical Nurses (LPN)
• Occupational Therapists (OT)
• Physical Therapist Assistants (PTA)
• Speech-Language Pathologists (SLP)
• Aides
• Medical Social Workers (MSW)
• Case Managers

Charting System

Experience with EPIC charting system is preferred.

Equipment Used

No specific equipment skills are required for this position.

Floating Requirements

This position covers multiple locations in the Southern Black Hills region, including cities such as Hot Springs, Custer, Edgemont, Hill City, and Hermosa. Mileage costs are reimbursed at the current IRS standard rates.

Orientation

Orientation typically lasts approximately one week and includes training on the EPIC system, shadowing, and documentation processes.

Scheduling

All positions operate Monday through Friday. There is no weekend rotation or on-call duty. The office observes six major holidays.

Time-off requests require manager approval.

Scrub color: Any color is acceptable, or business casual attire is permitted. Denim and sweatpants are not allowed.
