Variable shifts available:

Monday to Friday: 8:30 AM - 5:00 PM
Tuesday to Saturday: 8:30 AM - 5:00 PM
Sunday to Thursday: 8:30 AM - 5:00 PM

Minimum Years of Experience

1 year of experience required.

Certifications/Requirements

General Certifications: BLS/BCLS required.

Job Description

This position requires a candidate with primary experience in Acute Inpatient rehabilitation and secondary experience in Intermediate Care Facilities (ICF) or Skilled Nursing Facilities (SNF). Responsibilities include:

Evaluating and re-evaluating assigned residents/patients.
Planning and implementing an ongoing therapeutic physical therapy program as prescribed by the attending physician or consulting specialist.
Providing consulting physical therapist services as needed.
Availability to work shifts, weekends, and holidays as necessary based on operational needs.

Equipment Utilized

Electronic Medical Records (EMR) system: Meditech.

Required Skills and Experience

A minimum of 6 months of work experience is required in at least one of the following settings working with adults:

Acute Care
Rehabilitation Hospital
Skilled Nursing Facility (SNF)

Specific Licensures and Certifications/Registrations Required

BLS certification is required.
