Physical Therapist (PT)

Job Summary

We are seeking a dedicated and compassionate Physical Therapist to join our team in providing exceptional care in a home health setting. This position involves delivering personalized treatment to patients in their homes, assisted living facilities, and other community settings.

Key Responsibilities

• Conduct evaluations and develop treatment plans tailored to individual patient needs.
• Provide ongoing assessments and adjust treatment plans based on patient progress.
• Supervise up to 2 Physical Therapy Assistants (PTAs) or Certified Occupational Therapy Assistants (COTAs).
• Manage a caseload of 3 to 5 patients daily, including Start of Care visits and follow-up evaluations.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Maintain accurate documentation and progress notes in adherence to regulatory standards.

Qualifications

• Active California Physical Therapist License.
• Basic Life Support (BLS) certification required.
• Experience with HomeCare Home Base (HCHB) software preferred.
• Minimum of 2 years of experience in a healthcare setting; community or home health experience is preferred.
• Good organizational, oral, and written communication skills.
• Current driver’s license and proof of automobile insurance are required for travel.

Work Environment

The role involves working in various home environments, which can sometimes be cramped, hot, cold, or cluttered. Physical Therapists are expected to complete their work within designated shifts and are required to wear professional attire.

Additional Information

• 1 weekend per month may be requested, but is not mandatory.
• Holiday work may be required, with flexibility allowed for time-off requests.
• A robust three-week orientation process will be provided, including online training and field precepting.
• Mileage may be reimbursed at the government rate with prior approval.
