An opportunity is available for an experienced Occupational Therapist to provide home health services. The role involves visiting patients in their homes to deliver tailored therapeutic interventions.

Job Details

• Unit Type: Home Health
• Shift Length: 8 hours
• Guaranteed Hours: 40 hours per week
• Work Schedule: Monday to Friday, 8:30 AM to 5:00 PM

Job Responsibilities

The Occupational Therapist will be responsible for:

• Conducting patient evaluations and revisits, averaging four visits per day.
• Developing and implementing individualized treatment plans based on patient needs.
• Collaborating with healthcare professionals to ensure comprehensive patient care.
• Documenting patient progress and modifying therapy plans as necessary.
• Managing a caseload that covers a defined geographic area.

Qualifications

• Must possess a valid California Occupational Therapy license.
• Two years of experience in a healthcare setting is required.
• Current Basic Life Support (BLS) certification.
• National Provider Identifier (NPI) number.
• Current driver's license and proof of automobile insurance.
• Home health experience preferred. Consideration may be given to Certified Occupational Therapy Assistants (COTAs) with relevant experience if no OT applicants are available.

Coverage Area

The position will involve visits in the northeastern Sacramento area, extending into Folsom and Woodland in Yolo County.

Reimbursement

Mileage will be reimbursed at the standard General Services Administration (GSA) rate with prior approval.
