Description
Job Overview
We are seeking a dedicated Physical Therapist to join our Home Health team. The ideal candidate will be responsible for providing high-quality therapy services to patients in their homes, focusing on rehabilitation and improvement of physical functions.
Key Responsibilities
- Manage and treat a patient caseload, averaging 6.5 patients per day.
- Perform evaluations, develop treatment plans, and execute individualized therapy programs.
- Conduct initial and ongoing assessments, including SOC, ROC, Recertification, and patient evaluations.
- Collaborate with healthcare team members to optimize patient outcomes.
Qualifications
- Current BLS certification required.
- Minimum of 1 year of experience preferred; new graduates are not currently being considered.
- Experience in treating patients with common diagnoses, such as CHF, orthopedic conditions, and pneumonia.
Work Environment
This position will primarily operate within the communities of Walnut Hills, Evanston, Norwood, Reading/Lockland, Elmwood, St. Bernard, Northside, Finneytown/NCH, Avondale, Spring Grove, Deer Park, and Roselawn.
Shift Details
- Five 8-hour shifts per week from 8:00 AM to 5:00 PM, including rotating weekends.
- Holiday rotation is required.
Additional Information
- Orientation period lasts 2 weeks with a preceptor.
- Company-provided phone and tablet for work-related communication and documentation.
- Dress code is scrubs or business casual.
Support and Supervision
You will report to a Clinical Manager or Educator available for support 24/7, ensuring that you have the assistance needed for effective clinical performance and patient care.
