Description
PT Float: Physical Therapist Home Health
Location: Newton, Massachusetts Employment Type: LTA Contract Length: 13 weeks Shift: 08:00 - 16:30
Role Summary
- Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities from injury, disease, or other causes.
- Patients range in age from birth to over 100.
- Collaborate with patients, families, physicians, and other allied health professionals to identify needs and coordinate services.
- Complete timely electronic documentation and interdisciplinary communication per agency policies and standards of practice.
- Travel to patient cases as assigned.
- Weekend rotations occur once every 6 weeks.
Required Qualifications
- Valid Physical Therapist license in Massachusetts. Massachusetts conditional licenses are not accepted.
- At least 1 year of Physical Therapist experience, with experience within the last year.
- Experience in a home care setting.
Optional Items
- Chest X-ray documentation, only if a TB test is positive.
- TB symptom survey, only if a PPD is positive, completed before start and updated annually.
- Legal name change documentation, if credentials do not match the candidate's current legal name.
