Role Details

• Call: 2-4 days a month, which will require a holiday per contract

Required Qualifications

• Basic Life Support (BLS) certification from the American Heart Association, required for all direct patient care personnel
• Certified Surgical Technologist (CST) certification with at least 1 year of experience, including experience within the last year

• Specialty-specific competency testing: specialty exam completed within the past year

Health and Immunizations

• TB screening: 2-step PPD, QuantiFERON, or T-SPOT within 1 year of start. If positive, a chest X-ray within 1 year of start and a TB questionnaire are required. (The source gives a conflicting 5-year window for some regions.)
• Tdap within the past 10 years. Declination accepted only where the assignment does not involve pediatric interaction; agency declination form accepted.
• COVID-19: proof of September 2025 booster, declination, or exemption. Declination not accepted in CA or IL. End date October 2026.
• Influenza: proof of vaccine, declination, or exemption for the 10/1/2025-4/30/2026 season. Declination not accepted in CA or IL.
• MMR: 2 vaccination dates or primary-source IgG titer
• Varicella: 1 vaccination date or primary-source IgG titer
• Hepatitis B: 3 doses or declination
• Fit test: any mask model and brand, within 1 year of start
• Physical or health statement from an exam within the past year
• Color vision test
• 10-panel drug screen within 30 days of start, preferably within 14 days, resulted with MRO signature before start. Must include cocaine, amphetamines, opiates, 6-monoacetylmorphine, propoxyphene, PCP, barbiturates, benzodiazepines, methaqualone, and methadone. THC not included.

Background and Screening (within 30 days of start unless noted)

• Federal, nationwide, and county criminal check covering the past 7 years, plus SSN verification
• NSO (National Sex Offender) registry check. Michigan also requires the statewide registry.
• EPLS (SAM.gov) and OIG exclusion checks, with all aliases searched
• OFAC check, completed within 30 days of start
• Highest education verification through a third-party screening report (if the job requires specific certifications, those must be provided at onboarding)
