Role Summary

• Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes.
• Patients range in age from birth to over 100 years.
• Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate services.
• Use thorough and timely electronic documentation and interdisciplinary communication in line with agency policies, procedures, and standards of practice.

Required Qualifications

• Valid Physical Therapist license in Massachusetts
• At least 1 year of Physical Therapist experience, completed within the last year
• Experience in a home care setting

Schedule

• Weekend rotations occur once every 6 weeks.

Onboarding and Credentialing Requirements

• Licensure verification: Primary source verification of professional license within 30 days of start and updated upon expiration. Massachusetts conditional licenses are not accepted.
• Background checks:

  • Criminal background check within 45 days prior to start, including county, international, and SSN trace searches for the past 7 years
  • Massachusetts CORI within 30 days prior to start
  • National sex offender check within 30 days of start and annually
  • Exclusion checks (OIG, GSA, MassHealth, and NH Medicaid) within 30 days of start and annually
  • Motor vehicle record check within 45 days of start
• Health and immunizations:

  • N95 respirator fit test within 365 days of start and annually, completed off-site with no religious or medical exemptions (updates may be done on Martha's Vineyard or Nantucket if the assignment extends beyond the original term)
  • Respirator medical clearance attestation
  • MMR documentation or proof of immunity
  • Influenza vaccine, effective 10/4/2026 for new travelers and due by 11/13/2026 for travelers on assignment; no exemptions or waivers; self-administered flu mist is not accepted
  • TB test (skin test or negative QuantiFERON or T-spot) within 3 months of start
  • Physician health statement signed by an MD, DO, NP, or PA within 365 days of start
  • If TB test is positive: negative chest X-ray and TB symptom survey
• Other:

  • Nurse Aide Registry check within 30 days of start (required by the Massachusetts DPH for all employees)
  • Education verification (highest level achieved)
  • Badge photo meeting the specified guidelines
  • Confirmation of assignment form upon acceptance
• Submission items:

  • Resume covering the past 7 years, with city and state for all work and education
  • 2 references within 4 years, at least 1 from a supervisor or above
  • Skills checklist completed within 1 year
  • Candidate submission coversheet
• Optional: Name change documentation, if applicable
