PT Outpatient

Assignment Details

• Travel/Contract role in Valdez, Alaska
• Contract length: 26 weeks
• Start date: ASAP
• Day shift, 4x10 hours, 7:00-17:30
• Traveler’s choice: 5x8 or 4x10 days
• Open to accommodating a block schedule
• Guaranteed hours: 72 hours per 2 weeks
• Can be called off for 1, 8-hour shift per 2 weeks

Required Qualifications

• Valid Physical Therapist license in Alaska
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• One completed travel assignment
• BLS
• Experience in an Outpatient setting
• Experience in an Acute Care setting
• Experience in a Long Term Care or Skilled Nursing setting

Work Setting

• Department serves hospital (IP and swing bed), LTC, and outpatient

Float / On-Call

• Does not float within scope to meet facility needs
• No on-call required

Housing

• Housing is provided during the assignment
• Typically 1-bedroom apartments
• Available for move-in no earlier than 72 hours before contract begins and no later than 24 hours after contract ends
• Pets are not allowed
• Additional family members are not common and must be approved
• Housing must be cleaned upon departure or additional cleaning fees may apply

Other Notes

• Local candidates are not accepted; permanent address must be at least 50 miles from the facility
• Candidates previously employed as permanent staff for any Providence facility must be separated for at least 3 months before returning as a traveler
• Time off is approved on an individual basis upon offer
