Schedule

• Day shifts, 5x8 hours (40 hours/week, 8 hours/day)
• Shift options: 8:30 to 17:00 or 11:00 to 19:30
• Shifts are variable depending on census

• First-time travelers are accepted.

Required Qualifications

• Valid California Physical Therapist (CA PT) license
• Physical Therapist experience of 2 years
• Physical Therapist experience within the last year
• Basic Life Support (BLS) certification issued by American Heart Association or American Red Cross
• Experience with Epic

Clearances and Screenings

• 7-year background check, completed within 6 months of start
• Sex offender registry search, completed within 90 days of start
• FACIS Level 3 search within 2 weeks of start, with monthly updates after start
• Federal exclusion database checks (EPLS/SAM, HHS/OIG, OFAC)
• State Medi-Cal Suspended and Ineligible Provider List search
• Negative 9-panel drug screen within 6 months of start (no marijuana/cannabinoids on new screens)
• Education verification of highest level achieved (not required for high school)
• Primary source online license verification within 30 days of start
• NURSYS verification within 30 days of start (for RN, LPN/LVN, and AP roles)

Health Requirements

• Physical exam within 1 year of start
• TB screening questionnaire within 90 days of start, renewed annually
• Negative TB test (QuantiFERON or TSPOT within 90 days of start, or 2 PPDs) or negative chest X-ray, renewed annually
• Measles, mumps, rubella, and varicella immunity (titer or vaccination records)
• Hepatitis B immunity (titer, vaccine series, or signed declination)
• Tdap vaccination within 10 years of start
• Seasonal flu vaccination or documentation during flu season (November 1 to April 30)
• In-person color vision exam (online exams not accepted)
• Respiratory safety clearance, renewed annually
• Fit test within 1 year of start, renewed annually

• Employment application showing 7 years of work history with no gaps
• Work history verification for up to 7 years; gaps over 30 days must be explained
• 2 professional references required
