Description
Hospice RN
Location: Westover, West Virginia Contract Length: 13 weeks Shift: Days, 8-hour shifts, 40 hours/week (Sunday through Saturday, 5 shifts/week)
Required Qualifications
- WV nursing license
- BLS (AHA) certification
- Hospice experience
- Facility-specific training
Patient Population
- Mostly adult patients, with occasional pediatric patients
- Common diagnoses: post-COVID pneumonia, wound care, IV antibiotics, CHF exacerbation, COPD exacerbation, post-op joint replacement
Responsibilities
- Home health visits for general assessments
- Education on medications and disease processes
- Teaching caregivers how to provide care after home health discharge
- Monitoring the illness for which home health was ordered
- Possible wound care and IV infusion
- No additional job duties or utilization review
Equipment in the Home
- Oxygen and nebulizer are typically present
- Some patients already have a CPAP or BiPAP
Visit Expectations
- Approximately 6 routine visits per day
- Visit counts: routine = 1, SOC = 2, recert and OASIS discharge = 1.5
- Goal of about 30 visit-equivalents per week
- SOC and other OASIS visits may be discussed
Support and Coverage
- Home health aides are available during the day on weekdays
- Administrative on-call coverage after hours and on weekends for questions and concerns
- Most patients are seen Monday through Friday, 8:00 AM to 4:30 PM, with some evening and weekend visits as needed
Weekend and On-Call
- A weekend nurse is currently scheduled, so this role covers on-call duty
- On-call responsibilities include seeing patients discharged unexpectedly from the hospital or those needing an as-needed visit
- An on-call schedule covers after-hours patient needs
Holidays
- Holidays are divided evenly among staff: one major and one minor holiday
Floating
- No floating
Orientation
- Staff orientation, including a computer trainer
- Field orientation completed with a field or charge nurse
Other
- Royal blue scrubs
- Candidates may submit to only one position at a time within the health system
