Role Summary

The physical therapist assesses patients' ability to move and perform daily functional activities, identifies deficits, and develops individualized, patient-centered treatment plans to reduce pain, restore function, prevent disability, and preserve mobility.

Key Responsibilities

• Perform patient evaluations, including history, systems review, and selection and administration of tests and measures.
• Establish individualized plans of care in line with the evaluation findings and physician orders, and reassess patients' response to treatment.
• Initiate and manage discharge planning from the first visit through treatment.
• Supervise and delegate to ancillary staff and students in line with state law and department policy.
• Complete accurate, timely documentation that meets accreditation and insurance requirements.
• Communicate with patients, families, caregivers, and the health care team using AIDET, and provide education to meet identified needs.
• Participate in professional development, quality improvement activities, and safety practices, and function within the scope of practice defined by the state practice act.
• Perform other related duties as required and directed.

Requirements

• Valid Physical Therapist license in Texas
• Minimum of 2 years of experience as a Physical Therapist
• Physical Therapist experience within the last year
