Description

Travel LPN – Home Health

We’re currently seeking a Home Health LPN to join our amazing team.

Details of the opportunity:

Anticipated start date: ASAP
Assignment Length: 13 weeks
Setting: Behavioral Health

Daily Range of Responsibilities:

Perform in-home patient assessments and OASIS documentation
Administer medications, treatments, and wound care
Educate patients/families on home care and safety
Coordinate care with interdisciplinary team
Prioritize independent clinical decision-making in patient homes
Utilize Home Base Home Care software for documentation and care coordination

Job Qualifications:

Graduate of an accredited Nursing program
Active LPN license in the state of practice (or compact license)
Current BLS certification
Specialty Certifications (preferred): Wound Care, IV Therapy, OASIS Documentation, Home Base Home Care Charting
Experience: 1+ year home health experience
