Travel Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) – Long Term Care (LTC)
Location: Claremont, NH
Assignment Duration: 13 weeks
Start Date: 10/14/2025
Shift: Nights, 12-hour shifts
Weekends/Holidays: Every other weekend and holiday coverage required
On Call: None

Job Summary:
The LVN/LPN provides skilled nursing care to residents in a long-term care facility, observing changes in mental and physical well-being, administering medications and treatments, performing wound care, supervising LNAs, assisting with admissions, discharges, and transfers, and supporting the overall operation of the assigned unit. The nurse will collaborate with physicians, nursing supervisors, and direct-care staff to ensure high-quality resident care.

Key Responsibilities:

• Observe each resident and report pertinent changes to the Supervisor.

• Accurately transcribe and carry out physician orders.

• Administer medications, oxygen, and gastrostomy/jejunostomy feedings per facility guidelines.

• Perform wound care and manage wound vacs.

• Supervise LNAs and assign tasks according to abilities and licensing scope.

• Monitor unit documentation, infection control, and overall safety.

• Facilitate physician rounds and 60-day resident reviews.

• Communicate with residents’ families regarding care, concerns, and consent.

• Complete shift reports, review new orders, and maintain consistent communication with Nurse Manager/Nursing Supervisor.

• Assist with staff orientation, continuing education, and in-service training.

• Follow state and federal regulations for controlled medications.

Submission Requirements:

• 3+ years current experience (within the last 6 months) – REQUIRED

• Willingness to float where experienced – REQUIRED

• IV certified (or willing to obtain during onboarding) – REQUIRED

• Previous travel experience – Highly Preferred

• Rural/small facility experience – Highly Preferred

• BLS required
