What You’ll Do

Perform in-home assessments and design personalized therapy plans.
Deliver skilled physical therapy treatments and track progress.
Complete accurate OASIS and clinical documentation.
Collaborate closely with physicians, caregivers, and the care team.
Provide mentorship and guidance to Physical Therapy Assistants.
Manage your own schedule and patient appointments with flexibility and independence.

What We’re Looking For

Current State Physical Therapy license.
Graduate of an accredited Physical Therapy program (APTA or equivalent).
Strong communication, organization, and time-management skills.
Home health or geriatric care experience a plus—but passion and compassion are essential.
