• Applicable recent experience in this work setting

• Equipment/Procedures: EPIC, Medication Reconciliation for discharge medication list, Antimicrobial Stewardship, Pharmacokinetic Dosing Management/Monitoring, Anticoagulation Dosing Management/Monitoring
• Experience: 1 year
