HumanEdge Health is seeking a qualified Travel LMSW for a long-term assignment supporting the Clinical Care Management team in Valhalla, NY. This is a full-time, weekday position offering consistent hours in a clinical care environment.

Assignment Details
Location: Valhalla, NY
Start Date: ASAP
Duration: 30 weeks
Schedule: Monday–Friday
Hours: 8:00 AM – 4:30 PM

Department: Clinical Care Management

Role Overview

• Provide clinical social work services within a care management setting

• Coordinate care planning and support patient discharge and transitions of care

• Collaborate with interdisciplinary healthcare teams

• Assess psychosocial needs and connect patients with appropriate resources

• Maintain accurate documentation in accordance with regulatory standards

Requirements

• Master’s degree in Social Work (MSW) from a CSWE-accredited program

• Active New York State LMSW or LCSW license and current registration required

• Strong clinical assessment, communication, and documentation skills

• Healthcare or care management experience preferred
