Minimum Requirements:

• Pharmacist License for KY or Compact as applicable
• Applicable recent experience in this work setting
• Minimum 1 Year of experience
• Commitment to quality care and adherence to client legal and protocols
• Certifications: EPIC experience
• Equipment/Procedures: Medication Reconciliation for discharge medication list; Antimicrobial Stewardship; Pharmacokinetic Dosing Management/Monitoring; Anticoagulation Dosing Management/Monitoring
