HumanEdge Healthcare Staffing is seeking an experienced Registered Nurse (RN) Clinical Manager for an exciting leadership opportunity with a leading healthcare organization in Brooklyn, NY, near Rockaway Parkway/Canarsie.
This is an excellent opportunity for an experienced RN Supervisor, Nurse Manager, Home Care Clinical Manager, PACE RN, or Care Management RN who wants to step into a leadership-focused position with a consistent weekday schedule.

What You'll Do
As the RN Clinical Manager – Care Delivery, you will help oversee clinical operations, participant care, nursing staff, and interdisciplinary care coordination.
Key responsibilities include:

Oversee individualized participant care plans and clinical outcomes

Lead and participate in Interdisciplinary Team (IDT) meetings
-
Coordinate care with nursing, physicians, social work, rehabilitation, home care, and other disciplines
-
Supervise, coach, mentor, and evaluate clinical staff
-
Assist with staffing, clinical assignments, and coverage
-
Review participant charts and clinical documentation for accuracy and quality
-
Collaborate with clinicians to review UAS assessments
-
Monitor care management activities and participant responses to interventions
-
Coordinate transitions between hospitals, rehabilitation facilities, nursing homes, home care, and community settings
-
Oversee incident follow-up and required clinical documentation
-
Participate in interviewing, onboarding, staff education, and performance management
-
Support regulatory compliance and clinical quality initiatives
-
Serve as a clinical leadership resource for nursing staff and the interdisciplinary team
The position may also serve as Director of Patient Services (DPS) for the LHCSA, as assigned.
What We're Looking For
Must-Haves
-
Active and unrestricted New York State RN license
-
Graduate of an accredited nursing program
-
Supervisory or nursing management experience
-
Minimum 2+ years of healthcare administrative/management experience
-
Strong clinical assessment and care coordination skills
-
Experience working with interdisciplinary healthcare teams
-
Excellent leadership, communication, documentation, and organizational skills

The client's qualifications specifically require supervisory experience and identify CHHA, LHCSA, acute care, medical-surgical, critical care, nursing home, and diagnostic/treatment clinic experience as relevant backgrounds.
