Physical Therapist - Home Care

Location: Wall Township, New Jersey
Employment Type: Clinical-Allied
Contract Length: 13 weeks

Job Summary

Provide professional rehabilitation therapy to patients in their homes by evaluating, developing, and implementing individualized, goal-oriented therapy programs. Ensure compliance with organizational policies and external regulations.

Key Responsibilities

• Complete comprehensive patient evaluations within specified timeframes.
• Develop individualized treatment plans with measurable short- and long-term goals.
• Prepare and maintain accurate documentation including progress notes, reassessments, and discharge summaries.
• Ensure documentation supports the need for continued skilled care and is submitted timely.
• Follow physician’s plan of care regarding frequency and duration of services.
• Coordinate care as a liaison among physicians, managed care providers, and healthcare team members.
• Supervise Physical Therapy Assistants (PTAs), assessing their ability to perform interventions and ensuring timely co-signatures.
• Educate patients and caregivers on home programs and provide comprehensive education tailored to their needs.
• Build rapport with patients to foster an environment conducive to goal achievement.
• Independently meet productivity requirements consistently.
• Participate in discharge planning and address any issues during the patient’s care.
• Manage scheduling accurately and respond promptly to communications.
• Document case communications securely throughout the patient’s care.
• Ensure discharge summaries reflect patient status, outcomes, and recommendations for further care.
• Perform other duties as assigned, demonstrating flexibility.
• Adhere to organizational competencies and standards of behavior.
• Physically able to lift and push/pull at least 50 lbs and stand for a minimum of 8 hours daily.

Required Skills & Experience

• Minimum of 6 months of physical therapy experience.
• Proficient with PC and related software.

Education & Licensure

• Bachelor’s Degree in Physical Therapy (PT) required.
• Registered Physical Therapist license required.
• Basic Life Support (BLS) certification from the American Heart Association required.

Preferred Qualifications

• Doctorate degree in Physical Therapy preferred.
