Job Description

• Perform patient assessments including swallowing, communication, and cognitive abilities.
• Collaborate with patients, families, and physicians to develop and revise plans of care.
• Implement treatment programs to restore function and achieve maximum functional status.
• Provide discharge planning and referrals as needed.
• Use age-appropriate teaching methods for patients and families and document education tools.
• Maintain a safe environment by assessing social and clinical risks and following infection control measures.
• Communicate effectively with physicians, nurses, and other clinical disciplines.
• Comply with agency policies, documentation standards, and regulatory requirements.
• Participate in team and interdisciplinary meetings, prioritize schedules, and maintain visit productivity standards.
• Travel to patient homes and facilities as required using a personal vehicle.
• Assist with agency growth by orienting new employees, cross-training, and accepting additional assignments.

Required Qualifications

• Minimum of 1 year of experience in acute, subacute, or home care settings.
• Home health experience is required.
• Good customer service and communication skills.
• Use of personal vehicle for home visits is required.

Additional Information

• Must comply with paid time off request process and weekend/holiday requirements.
