Required Qualifications
- Valid Speech Language Pathologist license in New Jersey
- Speech Language Pathologist (1 year of experience)
- Speech Language Pathologist experience within the last year
- Experience in a(n) Acute Care setting
- Experience in a(n) Home Care setting
- Experience in a(n) Home Health setting
Key Responsibilities
- Assess patients’ appropriateness for home care and evaluate swallowing, communication, and cognitive abilities
- Develop and update care plans with the patient, family, and physician
- Provide treatment, education, discharge planning, and referrals as needed
- Maintain documentation, regulatory compliance, and interdisciplinary communication
- Travel to patient homes and facilities as needed
- Use a personal vehicle for home visits
