Job Opportunity: Home Care RN

Job Summary

Provide skilled nursing care to patients of all ages in their homes. Coordinate care with interdisciplinary teams, patients, families, and referring agencies. Responsible for assessment, planning, implementation, documentation, and evaluation of patient care plans to ensure quality and continuity.

Key Responsibilities

• Assess home health and hospice patients in stable and emergent situations, addressing physical, psychosocial, and environmental needs.
• Develop, implement, document, and evaluate individualized Plans of Care (POC).
• Collect and incorporate data on physical, psychological, social, spiritual, and cultural factors into care plans.
• Maintain accurate and up-to-date patient records reflecting all visits, contacts, problems, plans, and goals.
• Apply infection control measures to protect patients and staff.
• Educate patients and families on care, medications, diet, home safety, and promote self-care practices.
• Supervise and provide clinical direction to home health aides, STNAs, and LVNs.
• Assess and manage patient pain, including education on pain management techniques.
• Arrange referrals to appropriate agencies as needed.
• Monitor cases for compliance with third-party payer requirements.
• Provide on-call services as assigned.
• Serve as liaison among patients, families, healthcare team members, administration, and physicians.
• Participate in interdisciplinary team meetings.

Required Qualifications

• Valid Registered Nurse (RN) license in New Jersey
• Minimum of 2 years of Home Health nursing experience, including experience within the last year
• Associate of Science in Nursing or Associate Degree in Nursing
• Basic Life Support (BLS) certification
• Experience in a Home Health setting
• Competency in venipuncture and IV care
• Competency in performing phlebotomy for outpatient lab testing
• Experience with Homecare Homebase software
• Experience with OASIS documentation

Preferred Certifications

• Certified Hospice and Palliative Nurse (CHPN).

• Shift: AM
• Hours per day: 7
• Hours per week: 38

Contract Length

• 13 weeks (approximately 3 months)

• Prior travel experience is not explicitly required.
• Must be able to perform on-call duties as assigned.
