Shift: 1st shift
Hours: 8 hours/day, 40 hours/week

Job Summary

Evaluate and treat occupational therapy patients according to physician’s orders.

Key Responsibilities

• Perform initial patient evaluations using appropriate tests and measures.
• Identify impairments and functional limitations.
• Establish initial treatment plans with patient/caregiver involvement.
• Administer treatment strategies based on physician orders and treatment plans.
• Monitor and document functional improvements and goal attainment regularly.
• Update treatment plans and goals per department standards.
• Complete documentation timely and accurately, reflecting skilled therapeutic interventions.
• Maintain productivity standards and use non-productive time effectively.
• Comply with Home Health Care rules, regulations, and National Patient Safety Standards.
• Utilize resources responsibly and participate in department performance improvement activities.
• Lymphedema experience is required.

Required Qualifications

• Valid Occupational Therapist license in Wisconsin
• BLS certification issued by the American Heart Association
• Lymphedema experience

Additional Information

• Workdays: Monday through Friday, 8 AM to 4 PM.
