Job Description

• Position in Labor and Delivery unit (6 East) at Ascension Saint Alexius Medical Center.

Required Credentials and Certifications

Licenses & Professional Credentials (Required for Submission)

• Current nursing license with primary source verification within 30 days prior to start.
• Nursys verification within 30 days prior to start.
• Driver's License/State ID (must not be expired and must match address on file).
• Resume/Profile submission.
• At least two professional references prior to first assignment.

Certifications (Required for Onboarding)

• BLS (American Heart Association)
• ACLS (American Heart Association)
• NRP (Neonatal Resuscitation Program, AHA/American Academy of Pediatrics)

Exams and Testing (Required for Onboarding)

• Ascension Pharmacology Exam (minimum 80%, updated every 3 years)
• Ascension Specialty Exam (minimum 80% or national benchmark, updated every 3 years)
• Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics; minimum 80%, updated yearly)
• Ascension Cerner Test-Out (Cerner test results and training checklist with signed attestation)
• Ascension Skills Checklist (clinical skills, completed within one year of start, updated annually)

Onboarding Requirements

• Ascension Onboarding Requirements Education Attestation (6 hours pre-hire training)
• Ascension Nursing Contract Worker Onboarding (completed onsite)
• Ascension Biometric Consent
• Ascension Nova StatStrip (pre-hire modules for non-allied positions)
• Ascension Badge Photo (forward-facing headshot with white or blue background, no alterations)

Health and Immunization Requirements (Required for Onboarding)

• MMR: Documented proof of two vaccines or positive IGG titer (no declinations accepted).
• Varicella: Proof of two vaccines, serological immunity, or history verified by healthcare provider (no declinations accepted).
• Tdap: Valid within 10 years (no declinations accepted).
• Hepatitis B: Documentation of vaccination series completion or signed declination.
• Influenza Vaccine: Optional, but if declined during flu season (Oct 1 - Mar 31), mask wearing required.
• Fit Test: Required for all travelers; preferred prior to start, can be completed onsite.
• 2 Step PPD or IGRA (Quantiferon/Tspot): Required with specific timing prior to start; TB questionnaire updated annually.
• Positive PPD requires negative chest X-ray within one year and annual TB questionnaire.
• Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.
• Ascension 12 Panel Drug Screen: Within 30 days prior to start, updated annually.

Background and Verification (Required for Onboarding)

• Criminal Background Check including Social Security Trace, Employment Verification, OIG Search, National Sex Offender Registry (updated annually or if break in service > 90 days).
• EPLS (Excluded Parties List System) check within 90 days of start.
• Education Verification relevant to clinical work within 90 days of start.
• Employment Verification for past 7 years, updated annually or if break in service > 90 days.
• Confirmation of Assignment documentation upon initial assignment and each extension.

Optional / Additional Information

• Ascension Unit Specific Checklist (optional for onboarding, uploaded by unit leader).
• Influenza Vaccine Exemption/Declination accepted with mask requirement during flu season.
• Hiring Manager Evaluation (post-assignment).
• Legal documentation of name change (if applicable).
